New Patients

Anyone wishing to register with the practice must live within a 10 mile radius of the Surgery.

You can collect the New Registration forms from Reception or click on one the appropriate links below. Please note: You must complete either Form 1 or Form 2, and Form 3 must be completed by all patients wishing to register with our practice.

1. HS200: Patients who are registered with another practice in Northern Ireland

If you move or change any of your personal details (e.g. address, surname, phone number) please help us by informing us so that your records can be updated. 

Failure to do so could result in delays or prevent us from contacting you or a relative in an emergency.

Disabled patient facilities are also available at this practice.

(Car park space, ramp, toilet etc).
